By completing this form, I authorize Cookies Fabrics to service and/or repair my sewing machine. Our goal is to care for your machine as if it were our own. While every effort is made to handle each machine with the utmost care, I understand that Cookies Fabrics is not responsible for pre-existing conditions or unforeseen issues that may arise during the normal process of inspection and repair. I understand that I will be notified when my machine is ready for pickup, and I agree to pick it up within 90 days, unless other arrangements have been made in writing. If a machine is not picked up within 90 days and we are unable to reach you, it may be considered abandoned. In that case, I agree that Cookies Fabrics may take appropriate steps, including the sale of the machine, to help recover the cost of service, repairs, and storage.
